On December 3rd 1962 Vulcan XA894 was destroyed on the ground while performing an engine test of the TSR2 engine. During a full power test (with reheat) the rotating turbine disk came off, hit the ground, hit the fuselage, hit the ground again, smashed through the wing of the aircraft (setting the fuel tanks on fire) and flew half a mile to the end of the runway where it just missed the new Bristol 188 prototype. The crew of the Vulcan all escaped unharmed, however while fighting the fire a brand new fire engine was also destroyed when it was engulfed in burning aviation fuel.

Those, while disasters, were not all that dangerous. The leaks were minor and all the radiation was kept safely in the containment facility built into the rectors. It's just that the soviets were incompetent!

I know I've mentioned this before but I have an uncle who works at a nuclear power station and naturally there are warnings up all over the place about what not to do (most are real ones but some are joke ones just to lighten the mood). In the control room there was a big poster up with the following on to discourage people taking shortcuts or getting complacent.

'Nature doesn't care how many times you did it right, do it wrong this time and...
Everyone is dead,
The reactor is on fire,
And the countryside is covered in radiation!

It sort of stuck with me.
